How We Help

Whether you are navigating a serious illness or caring for someone who is, you have a lot on your mind. We’re happy to answer any question you have, at any time. You don’t have to sign up for anything to start a conversation.

When is it time?
If you believe a loved one may benefit from hospice or palliative care, call as soon as possible. The earlier we are involved in care, the better the quality of life and care for both patient and family. Hospice and palliative care programs both provide an extra layer of support as you navigate a serious illness.

What program is right for me?
Whether hospice or palliative care is a better fit depends on many factors, including what your most pressing needs are, details regarding medical eligibility, and your family’s comfort level. Our job is to make that process as easy for you as possible by working with you, your loved one, and your healthcare providers to make the right choice.

Does it cost anything?
The cost of hospice care is covered by the Hospice Medicare Benefit, Medicaid, private insurance, and donations to our non-profit organization. Our palliative care program is fully donor supported. We provide care for all, regardless of insurance status or ability to pay.

What are my next steps?
We recommend contacting us to learn more about programs and options. You can do that here, by calling us at 859-986-1500, or via email to hospice@hospicecp.org. Once you’re ready, we can work with your loved one’s providers to review medical records and arrange for a home or facility visit from one of our nurses. The nurse will assess your loved one and answer your questions.

Home Hospice Care

Hospice is about living well and at home when a serious illness effects a family.  Hospice is for those with a serious illness who are not pursuing curative or aggressive treatment, who wish to be cared for at home rather than in a hospital or facility, and whose family and caregiver need medical and care-team support to make that possible. Hospice is designed to support you and your loved one for many months.

Hospice is for those who understand that, even when one can’t be “fixed” medically, how they feel and what they do each day still matters.

Benefits of Home Hospice Care

  • Nursing assistants who help with light housekeeping and personal care
  • 24/7 nurse availability
  • Nursing visits to manage pain & symptoms
  • Help accessing benefits and resources
  • Majority of supplies, medical equipment, and medications covered by the Medicare Hospice Benefit and delivered to your home
  • Care that supports your faith traditions, beliefs, or spiritual practices

Palliative Care

Palliative Care is a free program designed for patients with a chronic, progressive illness who are still seeking curative or aggressive treatment, but need in-home support to help manage their condition.

Palliative Care provides a physician, nurse, and social worker who help:

  • Address pain and symptoms
  • Ensure access to community resources
  • Arrange for support with light errands, companionship, or other needs
  • Support the patient and family
  • Navigate healthcare and insurance systems

Inpatient Care at our Compassionate Care Center

Our Compassionate Care Center is a free-standing, hospice inpatient facility that offers short-term care for patients and families.

Having our own facility means that we can offer patients and families specialized, short-term respite and inpatient care in a peaceful, home-like environment.

The Compassionate Care Center is available to our Home Hospice Care patients and families and to transfer patients from regional hospitals and other facilities who meet hospice criteria.

The Compassionate Care Center features a home-like, warm atmosphere and is easy to access for visitors.

Care Includes:

  • General inpatient care, when a brief inpatient stay is necessary to manage pain or symptoms that cannot be managed at home
  • Respite care, a stay of up to five days, when a crisis necessitates a break for the caregiver

Many patients return home after their stay at the Center and continue with Home Hospice Care.

You can ask about care and services anytime.

If you or someone you love could benefit from hospice, palliative, or grief care, contact us.

We can provide a free consultation or simply talk to you to learn more about your needs.
